Preparing for Your Visit

Preparing For Your Appointment

Once you have scheduled your appointment with one of our doctors, please read the information on preparing for your appointment.

Parent or Guardian:

Patients under the age of 18 must have a parent or guardian with them during their office visit.

Appointment Arrival Time

The time you should arrive at the office for your appointment is different than your scheduled appointment time. You may be required to complete new patient paperwork or provide personal identification and insurance information prior to being seen by your doctor. What information you should bring to your appointment:

  • Current photo ID
  • Current insurance card(s)
  • Insurance co-payment, co-insurance and/or deductible information, if applicable
  • Insurance referral, if applicable*
  • School/Auto information, if applicable

*Please verify that our office has received the referral prior to your appointment. Having a copy of your insurance referral is needed for your appointment.

Late Policy

Please understand that if you are late for your appointment, your appointment may need to be rescheduled.

Cancellation Policy:

We require 24-hour notice for appointment changes or cancellations. Same day cancellations are subject to a service charge. Review our Payment Policy.

Know Before Your First Visit

Verify the date and time of your appointment. You may be required to complete new patient paperwork or provide personal information prior to being seen by your doctor. Please arrive approximately 30 minutes prior to your appointment time.

Confirm the address and location of your appointment. The Pediatric Orthopedic Center has four convenient locations in New Jersey. Confirm with the front desk staff the office location for your visit.
