Once you have scheduled your appointment with one of our doctors, please read the information on preparing for your appointment.
Patients under the age of 18 must have a parent or guardian with them during their office visit.
The time you should arrive at the office for your appointment is different than your scheduled appointment time. You may be required to complete new patient paperwork or provide personal identification and insurance information prior to being seen by your doctor. Because of this, you may be asked to arrive up to 30 minutes prior to your scheduled appointment time. Please confirm your expected arrival time when scheduling your appointment.
What information you should bring to your appointment:
*Please verify that our office has received the referral prior to your appointment. Having a copy of your insurance referral is needed for your appointment.
Please understand that if you are late for your appointment, your appointment may need to be rescheduled.
We require 24-hour notice for appointment changes or cancellations. Same day cancellations are subject to a service charge. Review our Payment Policy.
